"Addendum" or "Addenda"

"Addendum" or "Addenda"

Assuming I include one additional page to my application, containing an explanation of two LSAT scores and discolsure of a traffic violation, is it correct to title the page "Addendum to Application" or "Addenda to Application".

Thoughts?

I know it's trivial, but one of those crossing t's and dotting i's thing.

"Addendum" is singular.  "Addenda" is plural.

I know.  But in my case, which is it?  One page with two "items".

Ever heard of Latin?  It's a language...most western European languages are based on it.

Yes, I have. In fact, I studied it for three years.

I simply wanted to know if the heading should be plural since there are two items or singular since it's one page.

It's an addendum.  A thing added. 

That's what I'm trying to determine.
