If you're applying to Duke and were ever on academic probation, they want not only an addendum but a letter from the school explaining the details of your academic probation. I am not sure exactly what they expect beyond "GPA below 2.0 = academic probation", but I anticipate this being a huge pain in the ass.

Wow do they all ask about probation?

The reason for my probation should be obvious.  In the early 1990's my grades fizzled out and after several semesters of probation I flunked out of school.  I was on academic suspension.
I met the requirements to return, and returned 10 years later (much more mature). 
Still it took 3 or 4 semester of part time straight A's before my probation was lifted (I had to pull up over a 2.5).
In recent years I've earned 69 credits with an average of 3.95 and finished my degree.
That GPA is NOT factoring in damage done in the early 90's.
Still, given all these A's in recent years, do you really think anyone will give me a hard time over the probation/suspension of 1993????

Length of time + Huge GPA turn around = Not as bad as you think.
