What do you do if you think LSDAS made a mistake in calculating your GPA? has anyone ever had this problem before?

Good luck.  I had an issue with LSDAS counting a failed placement exam as a failure in a full credit class bringing my GPA down about 0.2 points.  I sent an email and they responded with a letter which basically re-stated their position as published on their website. 

Go through the motions of sending an email to LSDAS/LSAC (don't bother with a phone call, the people who answer have no power to make decisions or changes regarding your account) and use that as supporting documentation in an addendum.  That's my plan.

Of course, your situation might involve an actual accounting error.  If this is the case, make sure your email includes details about the math.
