Evaluation Forms and LSDAS

Hi all,

I have a question about evaluation forms and recommendation lettes sent via LSDAS. After combing through the LSAC website, it seems they just have the barcoded forms (for generic and specific letters) which must be sent with a copy of the letter to LSAC and are then forwarded to all law schools. But what about those evaluation forms that ask the recommender to check off boxes comparing you to other applicants? I do not see those listed on individual schools' supplementary materials, nor do I see a general form for this on the LSAC website. Do these forms exist for law school apps? If so, where are they and is it preferable to have them filled out/where are they supposed to be sent to? Also, am I correct in assuming that all top law schools accept generic letters sent from LSAC?
