Format for addendums?

Do you just attach them, or should you at least write "Addendum" at the top? Should you be more specific, like "Convictions Addendum"? For having the reputation of being a meticulous profession, these law schools are pretty damn bad at being definite about this stuff. Thanks.

I put my name, SS#, and LSDAS Acct # at the top of the page, and I wrote "Addendum to Application Question # X".  That's just my style; I'm sure everyone does it differently.

yeah, but some addendums won't be in response to an application question.

Anyways, mine is titled "addendum" if that helps.
