I've been rejected by more than one school that gave me a fee waiver.  It's very possible.

Thanks for the info.  I guess my past experiences with some undergrad schools as money sucking mosquitos forced me to think law schools would never, ever waive the app. fee unless they really, really, really wanted you.

One factor in the USNWR ranking system is how many admissions offers a school gives out compared to how many applications it recieves.  The more selective they appear, the higher they rank.
