Disclosure statement

I had a misdemeanor incident when I was 15.  I know you should write a letter stating all the conviction you have had, but I was wondering if this should be done in a statement like form or simply listed, stating when, where, why you did it. Should the disclosure statement be separate from the addendum, or do you state it in the addendum as well.  If so,  how would you go about doing that? thanks.

re-read that question, i think it says if you were a minor not to disclose it, I may be wrong though.
