recommender has gone AWOL!!!

i have a question for y'all...i got a recommendation letter from a coworker about 2 months ago...at the time, I didn't know about the LSAC recommendation form...now I have called and emailed this person at least 5 times, and am unable to get in contact with him..what should I do? submit his rec letter without the LSAC form, and maybe include a note to explain the situation...???any suggestions would be appreciated

I don't know that they'll take it without the form.  They need the recommender's signature.  You can call lsac, and see what they recommend, otherwise I might start considering who else to ask for a rec.

There is no space for a signature on the LOR Form.  I think it is more for matching up the letter to your file.

There is a space, however, on the back of the envelope after they seal it.  If it isn't sealed by the recommender, it's no good because for all they know you invented your co-worker who is now missing.  I don't think they'll accept it, but if I'm wrong, good for you.
