I titled mine "Personal Statement". You'll be sending in lots of documents in your application - addenda explaining various issues, personal statement, resume, a second essay in some cases, etc. I think the adcoms will appreciate a descriptive title on each doc that allows them to easily differentiate between the different docs.
I know for sure that adcoms hate gimmicks: putting your PS into some poetic rhyme/meter, colored ink on colored paper, beginning the PS w/ a definition (e.g. "Webster's defines courage as ..."), and structuring your PS in the form of a legal pleading. The reason I would think they hate gimmicks is because they are irrelevant and get in the way of efficiently assessing the core characteristics necessary for evaluating one's application.
Similiarly, the practical benefits for adcoms in terms of organizing your application by titling your PS "Personal Statement" seem to outweigh any gimmicky novelty gained through a clever title.
