"To the applicant: The preferred method for submitting recommendation letters is through LSAC. This form should accompany recommendation letters submitted directly to our office on behalf of your application"

If you choose to submit recommendation through LSAC, do you still have to print out this recommendation form and sign it? Or LSAC will take care of everything?

No, LSAC takes care of everything. There is an LSAC LOR form that you need to print out and have your recommender sign and send it with the letter.
