Do I need to attach anything along with the cancellation form??

I'm about to send my score cancellation form, and I was wondernig if I need to attach a written statement of intent to cancel, or whether just filling in that cancellation form is good enough.
Can someone let me know?  Thanks. 

AND WHAT IS THE FAX NUMBER FOR CANCELLATION??  CAN SOMEONE PLZ TELL ME

IT STATES ON THE FORM:

"VALID CANCELLATION REQUESTS MUST INCLUDE YOUR STATEMENT THAT YOU WISH TO CANCEL YOUR LSAT SCORE, YOUR NAME, SSN, LSAC ACCOUNT NUMBER, THE TEST DATE, CENTER NAME, CODE NUMBER AND YOUR SIGNATURE" I THINK WHAT YOU DETACH IS THE ACTUAL STATEMENT THAT INCLUDES ALL THE NECESSARY INFO REQUIRED. I DO NOT BELIEVE YOU NEED TO SEND IN ANYTHING ELSE.

FAX NUMBER: 215-968-1277

QUESTION: WE HAVE UNTIL 11:59PM TODAY TO CANCEL CORRECT?? ??

What do i write on the statement?  Do i just write it on a separate blank paper, saying
"I, ___ wish to cancel my December 3rd LSAT score."

??

NO NO. I'M SAYING WHAT YOU ACTUALLY CUT OUT(DETACH) IS THE ACTUAL STATEMENT ITSELF. YOU DO NOT NEED TO ATTACH ANYTHING ELSE ALONGSIDE THAT FORM.

Yes, I went ahead and cancelled my score today.  Although I felt I did well leaving the test center, I don't think I could have scored high since I ran out of time in numerous sections.  I guess on approx. 20 questions.  About 8 from RC (the last passage), 10 from LR, and about 2 or 3 from AR.

Does one cancelled LSAT record give bad impression on the Law schools?  Would it affect my chances on getting admitted??
