Changing dates of attendance on transcript request form?

So I took post-bac classes at both my undergrad institution and a community college.  I called LSAC and they said I need to request transcripts for those classes as well.  I've been looking through my LSAC account and can't seem to find a way to edit the dates on the forms.  Will I need to create new request forms for those dates?  Some help please!
