GPA/Academic Probation Addendum

I have a very rough first draft of an addendum that explains my grades from my first school (I did terribly).

Any English majors or others who wish to be a brutal critic on grammar and content?

I have tried to make it as factual (read: no excuses/whining) as possible and still tell exactly what happened. I'm looking for a happy medium.

Readers who are students who have had to write something similar are also appreciated.

I'm in a similar situation and would be glad to trade with everyone!  I am in the process of writing mine now, so it's not complete but I'll be glad to review for now and pass mine on later this week/early next week.  I think this is one of the most stressful parts to me, trying to explain away "deficiencies".   ???

I tried to explain what the issue was in as brief and direct a manner as possible.  The addendum is just one more thing for adcomms to read, but I think it serves an important purpose: it lets you tell your story.  Just don't go overboard with details.  Keep it high level and clear to understand.
