I have had around 23 jobs in the past 10 years...if I disclose all of these will I be ok?

That does look suspicious, but not as suspicious as if you try to hide it. Generally, you disclose it all and be prepared to answer questions. I think trying to hide it carries more risk.

I haven't filled mine out yet. Is that how many years back they want you to go? 10 years?
I've had so many jobs in the past ten years I doubt I could remember them all.

They may question it. If so, answer the questions honestly. That's about it.
As for how long they want, depends on the state.

Disclosure is always the rule here; there are very few things that will make you fail C&F (corporate fraud, etc) if you disclose them, but a lot of things end up being significant black marks if you DON'T disclose them.
You'll be fine.

I have several morbid obese kids at my school- how in the world will they pass the "fitness" part of the test?
