Title on personal statement

Does anyone know the preferred way of notating each attachment, such as the personal statement.  Should I write Personal Statement in the header with my name?  Or put my name in the header and have Personal Statement as the title?  Should I have a creative title?  Thanks for any advice.

Personal Statment

     Daddy left home when I was five for a prostitute that serviced clowns in the circus.  After my Mom died of lung, skin, brain, and toe cancer, I was left on my own to raise eight younger brothers and a llama.  This situation was made more difficult by my lifelong addiction to crack and tacky wallpaper.  But these experiences have made me stronger, and as such I believe I am ready to enter law school.

Something like that.

I left personal statement in the header, indented on the right side.
You can't get accepted to a school if you don't apply

do we need a title?
I have come to the realization that no matter how much I bull there will always be someone who is a bigger *&^% than me!!
