Certification Letter?

I got my Wisconsin app ready today.  It says if you apply online you have to print/mail a certification letter, but it doesn't offer a supplemental form you are supposed to use.  Does this mean I have to print page 4 of the app and mail that in?  Anyone know?

And...did anyone fill out the supplemental financial part of the Wisconsin app?  It only appears to be available online.  I don't know what the f the value of my car is.

You won't get the cert letter until you transmit the app.  It's not a supplemental form.  Once you transmit, an option will pop up to print the cert.
