Addendum needed?

My undergrad GPA was less than stellar, but not because of extenuating circumstances.  More like typical college student laziness.  Is an addendum really necessary though?  Or can I briefly mention in my PS that the student in his twenties is not the same individual who is now applying to law school (and entering his thirties!).  My attitude towards receiving a higher education is much different now.

So if an addendum is necessary, how do I go about attaching one?  Are there examples out there?  I'm curious about the format and how it complements the personal statement.
