Studying for the LSAT / Test Accommodations Good News/Questions

I have ADHD pretty bad, so I had to apply for accommodations for the Sep test.  I was granted the following:

-17 extra minutes for each timed section (time and a half)
-17 extra minutes for the written section
-15 minute break between sections 3 & 4

Is this normal?  Was I given the usual amount of extra time for someone with ADHD?  Do they usually give more or less?

Where do I go to print out the form?  Do I have to send it to the schools for them to send with the Transcript?  It just seems that some of these yahoos at LSAC are trying to make the application process as difficult as possible.  I mailed them an actual transcript that my school sent me (it was unopened), and they tell me that they couldn't take it that it had to be mailed from the schools them selves, WTF!

I tried looking on LSAC's website for the address to have my school send my transcripts to LSAC and couldn't find it, does anybody know where I can find it?

So to what address do I send have my school send it too? Thanks in advance.

Here's my situation.  I'll be getting my LG Bible in the mail on Thursday.  I won't be able to get the LR Bible until mid to late June.  Do I need more than 3 months to digest and implement effectively the principles taught in the LR Bible?  Could I digest its info in two weeks?
