Law School Applications / does this mean that i am hold/waitlisted status?

I recently received an application completion notice from the X law school. I mean this school was the first one i filed my application. According to my LSAC account, the LSDAS report date is 12/20/07.  anyhow, they told me that my file will be sent to the committee for review and told me that sometimes it can take a few months for the process

Am i on a hold list? or am i stretching the meaning out of the context
I am curious to know who makes the admission decision?

is it possible for applications not to be sent to the admission committe
for review? In other words, do law schools review all the applications?

it has been more than a week since i submitted my E-application.
I see that couple schools have not requested my LSDAS report.
is that because they are incredibly busy? should i contact the school
and urge them to act on my application?

i received an email from the law school saying that
my file was sent to the admissions committee for review.
For this particular school, they require three letters of reference.
but, i know for sure that they only have two letters.

is this administrative errors? what does this mean?
any ideas?

is it ok to use the letter of recommendation which is dated 2002.
I got the letter from the professor at that time,however.
 i did not apply to law school during that year. know what i mean..
