Just call them and let them know that you've made a mistake in filling out the forms and request to send them updated copies to replace the originals.  Most schools will either replace or include them in you file.  If you'd like you can write a short note explaining what you just said in your post.

I wouldn't stress about it - I'm sure adcoms see it every day.

I submitted on 10/27 as well - no report request yet.  My only guess is since I've indicated that I'm retaking in December, they will wait to get my report when December scores come out.  Could also be because I'm below their 25% LSAT at the momoment (but above 75% for GPA - I know, worst kind of splitter) :(

I applied on 10/27 and the still haven't requested mine, even though EA deadline was 11/1.  Just trying to figure out if they're taking their time on everyone, or it's because my numbers suck.

Has anyone done it? Anyone planning to do it?

Reading their Assitant Dean's comments and the write-up on their website, it almost sounds like it can hurt as much as help.  I mean, they say that they won't ask questions and see the inteview as your time to present your case for admission, plus at the end of the interview you provide a 15-min. writing sample on one of their pre-selected topics.

Not that it sounds too hard, just a lot more intense than what other schools are doing...
