I wrote my name, social security number and LSAC number on everything. I had no heading for my personal statement, for addendums, I titled them based on what the subject was (like Addendum to Question #2).

I don't know, one typo wouldn't be so bad, but totally ignoring their instructions re: font size, spacing, etc. seems worse because it either shows you didn't read the instructions or you didn't care enough about the school to follow them.

The LSAT is always part of the LSDAS report. And yes, you need a report for every school.

Butch, I would also calculate what your GPA would be based on those narrative grades and credit hours (using LSAC's system) and put that in the addendum.

Uh oh.... does it matter if a resume is in 10 point font?
I thought just the PS had to be 12 point font.

They won't really start reviewing your app until your file is complete, so I wouldn't worry about it. Just submit as soon as you can.
